This page describes the policy held by Pathways Child Psychology for the management of the practice's client information.

Client information

Client information is held in a secure location which is accessible only to authorised persons. The information on each file includes personal information such as name, address, contact phone numbers and other information which is relevant to the psychological service being provided. The patient/carer is responsible for informing Pathways Child Psychology any changes or updates of personal information.

Purpose of holding information

The information is gathered as part of the assessment, diagnosis and treatment of the client. The information is retained in order to document what happens during sessions and to assist with treatment.
